Endotrakeal Kaf İnflasyon Yöntemlerine Bağlı Yüksek İntraoperatif Kaf Basıncı İnsidansı ve Klinik Etkileri

GİRİŞ ve AMAÇ: Endotrakeal tüpe ait kafın en önemli fonksiyonu pozitif basınçlı ventilasyon sırasında kaçak oluşturmaması ve faringeal içeriğin aspirasyonunu engellemesidir. Endotrakeal tüp kafları için güvenli basınç aralığı genellikle 20 ila 30 cmH2O arasındadır. Kaf basıncındaki düşüklük hava kaçağına, gastrik ve orofaringeal içeriğin mikroaspirasyonu sonucu na¬zokomiyal pulmoner enfeksiyon oluşturabilme riskine sahiptir. Kaf basıncı mukozal perfüzyon basıncının üzerine çıkarsa tra¬keal mukozada kan akımı yavaşlar ve durur. Sonuçta Overinflasyon, boğaz ağrısı ve ses kısıklığı, trakeal rüptür, darlık ve trakeo-özofageal fistül gibi çeşitli komplikasyonlarla sonuçlanabilir.
Çalışmamızın amacı uygulanan bu subjektif yöntemin güvenli ölçüm değerlerini sağlayıp sağlamadığını kontrol etmek ve bu konudaki farkındalığın gözden geçirilmesidir.

YÖNTEM ve GEREÇLER: Yerel etik kurul onayı alındıktan sonra ASA I-IV risk grubunda 249 hasta dahil edildi. Rutin anestezi monitörizasyonu uygulanan hastalara rutin indüksiyon ve idamesi uygulandı. Tüm hastaların demografik verileri, tüp no, kaf pilot şişirme yöntemi ve şişiren kişiler kayıt edildi. Sonrasında 15. dakikada kaf basıncı monemetre ile ölçüldü ve kayıt edildi. Yüksek bulunan basınçlar normal aralığa kaçak olmayacak şekilde indirildi. Derlenme odasında ve 24. saatte boğaz ağrısı, ses kısıklığı ve yutma güçlüğü açısından komplikasyonlar kayıt edildi.
BULGULAR: Çalışmamıza 249 hasta dahil edildi. Hiçbir hasta güvenli kabul edilen 20 cmH2O’un altında kaf basıncına sahip değildi. Hastalar kaf basınçlarına göre Grup N (20-30 cmH2O), grup I (30-50 cmH2O), grup II (50-70 cmH2O) ve grup III (>70 cmH2O) olacak şekilde dört gruba ayrıldı. 249 hastanın 18’i Grup N (%7.2), 105 hasta (%42.1) grup I, 50 hasta (%20) grup II, 76 hasta (%30.5) en yüksek basınç grubu olan grup III’ te idi. Hastaların % 78’inde palpasyon yönteminin kullanıldığı bulundu (p<0.001). Postoperatif boğaz ağrısı derlenme odasında 8 (%3.2) ve 24. saatte 78 (%31.3) hastada görüldü. Derlenme odasında boğaz ağrısı yaşayan 8 hastanın hepsi en yüksek kaf basıncına sahip grup III’te idi (p<0.001). 24. saatteki boğaz ağrısı özellikle basınç artışı ile kolere seyrederken, Grup III’de 76 hastanın 61 (%80.2)’sinde pozitif olarak bulundu (p<0.001). Basınçtaki yükseklik postoperatfi dönemde ciddi oranda boğaz ağrısına neden oluyor. Bu süre anestezi pratiğinde rutin değerlendirme periyodu içerisinde olmadığından dolayı gözden kaçabilmektedir.
TARTIŞMA ve SONUÇ: Yapmış olduğumuz bu çalışma ile endotrakeal tüp kaflarının basınçlarının rutin kullandığımız yöntemlerle (palpasyon ve kaçak sesi olamayıncaya kadar) uygun şekilde ayarlanamadığı ve büyük çoğunlukla yüksek basınçlara neden olduğunu gördük. Ayrıca, bu yüksek basınçların da posteoperatif dönemde gelişen boğaz ağrısı ile ilişkili olduğunu ortaya koyduk.

Hıgh Intraoperatıve Cuff Pressure Incıdence Due to Endotracheal Cuff Inflatıon Methods And Its Clınıcal Effects

INTRODUCTION: The most important function of the endotracheal tube cuff is, it does not cause leakage during positive pressure ventilation and prevents aspiration of pharyngeal content. The low cuff pressure has the risk of air leakage, micro-aspiration of gastric and oropharyngeal contents. If the cuff pressure increases above the mucosal perfusion pressure, the blood flow of tracheal mucosa. Over-inflation, sore throat and hoarseness may result in various complications such as tracheal rupture, stenosis and tracheo-esophageal fistulae.
The aim of this study is to check whether this subjective method provides safe measurement values and to review the awareness of this subject.

METHODS: After the approval of the local ethics committee, 249 patients in the ASA I-IV risk group were included. Routine induction and maintenance of patients undergoing routine anesthesia monitoring was performed. Demographic data, tube number, cuff pilot inflation method and inflatable subjects were recorded. After 15 minutes, the cuff pressure was measured with the monometer and recorded. High pressures were lowered to the normal range without leakage. Complications in the recovery room and at the 24th hour for sore throat, hoarseness and dysphagia were recorded.
RESULTS: 249 patients were included in this study. Patients were divided into four groups according to cuff pressures: Group N (20-30 cmH2O), group I (30-50 cmH2O), group II (50-70 cmH2O) and group III (> 70 cmH2O). Eighteen (7.2%) patients, were in group N, 105 (42.1%) were in group I, 50 (20%) were in group II, 76 patients (30.5%) were in group III with the highest pressure group. Palpation method was used in 78% of patients (p <0.001). Postoperative sore throat occurred at 8 (3.2%) in recovery room and 78 (31.3%) patients at 24.hr after operation. 8 patients who had sore throat in the recovery room, were in group III (p <0.001). The throat pain at 24 hours was observed to be positive especially with elevated pressure and 61 (80.2%) of 76 patients were in Group III (p <0.001). The pressure in the postoperative period is a serious cause of sore throat. This period may be overlooked because it is not within the routine evaluation period in anesthesia practice.
DISCUSSION AND CONCLUSION: With this study, we have seen that the pressures of the endotracheal tube cuff cannot be adjusted by the routine methods which we use (palpation and minumun leak volüme methods) and this metods can cause high pressures. We also found that these high pressures were associated with sore throat that developed during the post-operative period.
