Ameliyat planlanan hastalar arasında bitkisel ürün kullanımı ve bitkisel ürünlerin nötrofil lenfosit oranına etkisi: Kesitsel araştırma

GİRİŞ ve AMAÇ: Bitkisel ilaçlar giderek daha fazla kullanılmaktadır. Literatürde dünya nüfusunun yaklaşık %80’inin bitkisel ürün kullandığını bildiren yayınlar vardır. Birçok hasta bitkisel ürünleri ilaç olarak kabul etmediği için geçireceği cerrahi öncesi bitkisel ürün kullandıklarını ifade etmemektedir. Bitkisel ürünlerin çok sayıda yan etkileri olduğu bilinmektedir. Çalışmanın primer amacı, ameliyat planlanan hastalar arasında bitkisel ürün kullanımının hazırlanan anket aracılığıyla araştırılmasıdır. Sekonder olarak ise son 1 ay içinde bitkisel ürün kullanan hastaların nötrofil lenfosit oranının diğer hastalarla karşılaştırılması amaçlanmıştır.
YÖNTEM ve GEREÇLER: Anestezi polikliğininde hastalara 11 sorudan oluşan bir anket 3 ay süreyle uygulandı. Anket tamamlandıktan sonra hastanın aktüel hemogramındaki nötrofil ve lenfosit düzeyleri kaydedildi.
BULGULAR: Anket toplam 445 hastaya uygulandı. Katılanlardan 159 hasta şu ana kadar bir şekilde bitkisel ürün kullandığını ifade ederken 39 hasta ise son 1 ay içerisinde kullandığını söyledi. Bitkisel ürün kullananların ve kullanmayanların eğitim durumunda istatistiksel farklılık yokken, yaş ve cinsiyet açısından farklılık vardı. 280 hasta daha önceki cerrahileri sırasında bitkisel ürün kullanımlarının sorgulanmadığını belirtti. Son 1 ayda bitkisel ürün kullananların nötrofil lenfosit oranları kullanmayanlarınkinden daha düşüktü.
TARTIŞMA ve SONUÇ: Bitkisel ürünlerin potansiyel problemleri arasında koagulasyon bozukluğu (kanama ve antikoagulasyon), kardiyovaskuler yan etkiler (hipertansiyon,ödem ve kardiyak ritm bozulması), su ve elektrolit bozukluğu (sodyum retansiyonu ve hipokalemi), hepatotoksite ve anestezik ajan etkisinde uzama (konfüzyon ve sedasyon), endokrin etkiler (hiperglisemi ve hipoglisemi) sayılabilir. Nötrofil/lenfosit oranı (NLO), tam kan testindeki nötrofil ve lenfosit değerleri kullanılarak hesaplanan ve günümüzde popülaritesi günden güne artan bir göstergedir. Anestezi uzmanları bitkisel ürün kullanımı hususuna daha fazla dikkat etmelidirler.

The usage of herbal products among patients scheduled for surgery and the effect of herbal products on neutrophil-lymphocyte ratio: a cross-sectional study

INTRODUCTION: Herbal medicines are increasingly used. There are publications in the literature reporting that approximately 80% of the world population uses herbal products. Since many patients do not accept herbal products as medicines, they do not state that they used herbal products before surgery. Herbal products are known to have many side effects. The primary purpose of the study is to investigate the use of herbal products among the patients planning surgery through a questionnaire prepared. Secondary, it was aimed to compare the neutrophil lymphocyte ratio of patients who used herbal products in the last 1 month with other patients.
METHODS: A questionnaire consisting of 11 questions was administered to the patients in the anesthesia policlinic for 3 months. After completion of the questionnaire, neutrophil and lymphocyte levels on the current hemogram were recorded
RESULTS: The questionnaire was administered to a total of 445 patients. 159 of the participants stated that they have been using herbal products so far, while 39 of them stated that they have been using them in the last month. While there was no statistical difference in educational status of users and non-users of herbal products, there was a difference in terms of age and gender. 280 patients stated that the usage of herbal products was not questioned during previous surgeries. Neutrophil-lymphocyte rates were lower in herbal products than in non-users.
DISCUSSION AND CONCLUSION: Potential problems of herbal products are coagulation disorder (bleeding and anticoagulation), cardiovascular side effects (hypertension, edema and cardiac rhythm impairment), water and electrolyte disorder (sodium retention and hypokalemia), hepatotoxicity and prolongation of anesthetic agents (confusion and sedation), endocrine effects (hyperglycemia and hypoglycemia). Neutrophil / lymphocyte ratio is an indicator that is calculated by using neutrophil and lymphocyte values and its popularity is increasing day by day. Anesthesiologists should pay more attention to the use of herbal products.
